At Oakridge Healthcare, we recognize
and value your privacy and are committed to protecting the
confidentiality of both your business' and your personal information.
By accessing, using and/or downloading materials from this
Site, you agree on your own behalf and on behalf of each entity
on whose behalf you act to the terms of this Privacy Policy.
Oakridge Healthcare Privacy
Principles
Oakridge Healthcare believes strongly in the protection of our
customer's privacy and we are committed to ensuring that your
commercial and personal information is appropriately safeguarded.
To this end, we have adopted the following privacy principles:
• We respect all of
our customer’s rights of privacy.
• We collect and use only the information needed for
legitimate business uses.
• We will not disclose identifiable commercial or
business information to any third parties unless there is
an appropriate legal or business justification to do so.
• If we contract with any other entity to perform
services which require that we share your information (for
example, credit reporting agencies or our underwriters)
we require that entity to maintain the confidentiality of
your information.
• We employ security measures that guard access to
and disclosure of customer information.
The collection,
use and disclosure of personal information is fundamental
to the credit and Healthcare business. As a result, Oakridge
Healthcare wants to inform those individuals with whom it does
business of how and why it handles personal information.
Personal information, for
the purposes of this Privacy Policy, means information that
identifies an individual. For example: an individual’s
name, birth date, address, age, income and other financial
information is personal information which Oakridge Healthcare,
(and other entities with which it shares credit risk) (collectively
“Oakridge Healthcare” “we” “us”
“our”) may collect, use and in certain circumstances,
where necessary, disclose, in the course of providing Healthcare
services and carrying on business. Personal information does
not include the name, business title, address, phone or fax
number of an employee of an organization.
This Privacy Policy may be
modified from time to time. An up to date version can be obtained
by contacting Oakridge Healthcare at the address set out below
in the section called “Contacting our Privacy Officer”.
1. Consent and the
Collection of Personal
Information Personal
information is typically collected by Oakridge Healthcare about
loan/lease applicants, debtors, lessees and guarantors for
credit/Healthcare purposes, such as: (i) to assess and process
credit/lease applications; (ii) to administer approved loan/leases;
and (iii) to enforce any obligations owed by applicants, debtors,
lessees or guarantors. Oakridge Healthcare consults its existing
files for these purposes. Oakridge Healthcare updates information
from time to time for these credit/Healthcare purposes.
Whenever practical, Oakridge
Healthcare collects information directly from the individual
concerned. In some instances and as is common in the underwriting
of credit risk and in the collection of debt, Oakridge Healthcare
verifies information and obtains information from independent
sources and organizations.
Oakridge Healthcare obtains consent
to the collection, use and disclosure of personal information,
except where inappropriate. Typically, we obtain authorization
from individuals to collect, use and, where necessary, disclose
their personal information for credit/Healthcare purposes on
the application for credit.
However, in many instances
Oakridge Healthcare’s business is conducted, and hence
personal information collected, through independent brokers
who act as intermediaries in obtaining credit or Healthcare services
from Oakridge Healthcare. These intermediaries are responsible
for their own privacy practices and for obtaining appropriate
consent to the collection, use and disclosure of personal
information. Individuals with questions pertaining to the
personal information handling practices of such an intermediary
should contact that organization directly.
2. Using Personal Information
Oakridge Healthcare uses personal
information in accordance with the authorization provided
by the individual to whom the information relates, in accordance
with this Privacy Policy, its contractual rights and obligations
and the law.
Personal information shall
not be used by Oakridge Healthcare employees and representatives
for purposes other than as is required in the performance
of their employment duties.
3. Accuracy of Your
Personal Information
We have procedures to ensure
that the information we collect and use is accurate, up-to-date,
and as complete as possible. However, we rely on the individuals
we do business with to disclose all material information and
to inform Oakridge Healthcare of any changes. With proof of entitlement,
an individual may request to access and, if applicable, request
to correct, personal information in Oakridge Healthcare ’s
custody. See the section called “Retention and Access
to Your Personal Information”.
4. Safeguarding Your
Information
We apply safeguards to our computer networks and physical
files and we restrict access to personal information to those
Oakridge Healthcare employees or authorized administrators who
need to know that information in order to arrange for, provide
or service loans/leases and to enforce the collection of debt.
5. Disclosure of Personal
Information
Personal information is sought from the individuals we do
business with and exchanged with third parties for credit
Healthcare purposes. Such third parties include references; personal
information and credit reporting agents and bureaus; and other
institutions with whom applicants, debtors, lessees and guarantors
may each have or expect to have financial dealings. Oakridge
Healthcare also shares information to combat fraud; where permitted
or required by law; or, at the request of government regulators.
Oakridge Healthcare sometimes
retains an affiliated company or an independent third party
(“authorized administrator”) to perform on Oakridge
Healthcare ’s behalf, certain functions in support of the
products and services Oakridge Healthcare provides. Such functions
could include arranging for funding, underwriting or sharing
of credit risk, administrative services or services in connection
with the collection of debt. Accordingly, in certain instances
these affiliates or third parties will be provided with personal
information to the extent that it is necessary in the performance
of those specific services. Oakridge Healthcare obligates these
affiliates and third parties to use and take steps to protect
personal information in accordance with the requirements of
this Privacy Policy.
Oakridge Healthcare does not sell
or disclose its customer lists for marketing purposes.
6. Retention and Access
to Your Personal Information
Personal information is retained only for so long as is necessary
to fulfill the purpose for which it was collected and to meet
legal and contractual obligations. If personal information
is no longer required, it will be destroyed or rendered anonymous.
If
an individual has a right of access, the individual or a person
authorized in writing to act on their behalf may, with satisfactory
identification and proof of entitlement, request access or,
if applicable, request a correction of personal information
which Oakridge Healthcare has in its custody.
The right to access is not
absolute. For example, there is no right to access and access
may be declined, if the information requested is subject to
a legal privilege.
A reasonable fee may be charged
in advance for copying and sending requested information requested
to which there is a right of access.
E-mails you send us are not
necessarily secure when they are transmitted to us. If your
communication is very sensitive, you may want to send it by
regular mail instead.
